I am thinking a tour guide for a day in Bangkok and around might be a good idea. We will be there near Christmas. Also, should we get shots? Like TThypoid? We are also going to Koh Chang.

You should take advice from your doctor about shots, but none are required, but each person has different health needs.
A tour guide within Bangkok is not necessary. Use the skytrain (BTS), subway (MRT) and Chao Phrya Express boats (like a public bus) to get you where you want to go quickly cheaply, and with some nice views too!
